本人， 授权及同意中国教育部留学服务中心（“中心”）就认证本人学历学位的有关事宜向相关学校或机构透露本人的个人资料并对本人取得国（境）外学历学位证书及高等教育文凭的情况进行核查。同时也授权并同意相关学校或机构（如英国高等教育数据核查平台（HEDD），美国全国学生信息中心（NSC）等）可就中心核查内容，向中心透露任何有关本人的所有记录及资料。
本人授权中心工作人员 向北京市出入境管理局核查本人留学期间出入境记录情况。
Declaration
I， authorize and consent to the Chinese Service Center for Scholarly Exchange （CSCSE）making any enquiries, which requires releasing my personal information, for purposes relating to qualification evaluation. I also authorize and consent to relevant schools/organizations (e.g., HEDD, NSC) to release all my record or information to the CSCSE as may be required for these enquiries.
